Most nurses remember the first time a child screamed through an injection they gave. It stays with you. Procedural pain management in children is a clinical skill — one that combines technical precision with empathetic care and, increasingly, the right tools. Understand the child's age and developmental stage


Pain response and coping ability vary enormously with age. Neonates and infants under 6 months respond primarily to physical comfort — swaddling, sucrose solution, and skin-to-skin contact with a caregiver during the procedure. Toddlers (1–3 years) are the most difficult age group: they have strong fear responses but limited ability to understand explanations. Minimise waiting, work quickly, and use vibration analgesia. Older children and adolescents benefit from being given some control — letting them choose which arm, counting down together, or explaining the procedure step by step.

Position matters more than you think

Evidence strongly supports using the 'comfort position' instead: the child sits upright in the parent's lap, held firmly but gently. This position reduces procedural anxiety, makes the child feel less helpless, and can measurably reduce the duration of post-injection crying. Whenever possible — and especially for vaccinations — use the comfort position.
